Information session on Respect Forms

Are you interested in an information session on Respect forms? A Respect form is a document that is created through conversations between a patient and one or more health professionals.  It records a patient’s preferences for resuscitation, care and treatment during an emergency or when you are unable to make or express choices for yourself.
